Permission Letter to Participate in a Web-based Project

Dear Parents/Guardians,

The students in Grade ______ at ________________________________________________ School are participating in an exciting Internet-based project, "Canadian Heroes and Heroines: Past, Present and Future".

Students are invited to research their Canadian hero or heroine and then create a website showcasing their accom[lishments. The work created by the students will be hosted on the St. Elizabeth School website in Ottawa, Ontario, Canada. (http://www.occdsb.on.ca/~sel/cahero/).

Your child is invited to participate in this web-based learning experience. He/she will work as a member of a team and may create art work, photos or writing which may be suitable for publication on the World Wide Web. If a student's work is chosen for posting on the website, only the first name and age will be used to identify the author.

If you have any concerns or questions, please feel free to contact me. Please sign the permission form and return it to school as soon as possible.

I give permission for my daughter/son ____________________ to participate in the web-based project "Canadian Heroes and Heroines". I understand that if her/his writing or art work is chosen for publication on the World Wide Web, no home address, telephone number, personal e-mail or last name will appear with the work. Only the first name and age will be used to identify the author. This will ensure privacy, but still give credit to the author. The work will appear as part of an educational resource with a copyright notice prohibiting the downloading of work without written permission.
